How To Make A Q&A In Your Story

Ello! I’ve seen many stories with this Q&A thing at the end of a story or the end of an episode, and I was wondering how I could make those? There are usually 5-10 questions/choices. Can I just use normal choices for this? Is there a template or something? Helpppp
