Hard to say. I was raised a Catholic, been agnostic for years, and only now I’m starting to catch up with God heyy 

I mostly consider myself a Christian because it’s familiar and easier for me to understand God that way but I’m not into all of the coded rituals, prayers etc. I consider my relationship with God a personal one that I can’t really fit into any organized religion. I simply strive for a deep and heartfelt connection with God in a more universal approach, however bold of a claim that is.
Also a few things on the poll. A Catholic is a Christian, just a specific kind. And if you included Catholicism I think you should include all kinds of Christianity, but also Buddhism, Islam, etc. So ‘Catholic’ can be together with ‘Christian’ imo.
Agnosticism and atheism are very different concepts. You are agnostic when you question the existence of (a) god, when it’s a constant dialog within yourself to determine whether it’s there or not for you. Being atheist means that you do not believe in the existence of any god and it’s quite a certainty. So… maybe don’t put them together? 
And as others said, there’s way more religions in the world that deserve recognition, so perhaps consider adding other options or another poll to include as many!
